This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, properS/ lines, streets, and unusual natural or topographic feature~ 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. STATE OF N~4 YORK) :.ss: COUNTY OF SUFFOLK) WII,IJAM WICEHAM, residing at Old Harbor BDad, New Suffolk, New York, being duly sworn, deposes and says that deponent is the attorney for Paln~r Ryland Griswold, the owner of the premises on the north side of West Noad, Fleets Neck Cutchogum, New York; which is being sold to Lea Ann Infranca. A copy of the sur~y of said property is attached hereto. Deponent is familiar with the property both practically and with the title. Although deponent does not have the actual date of constructi~ of the residence, the resid~_nce was on the property when Clintona R. Griswold cc~veyed the property to Palmer Ryland Gri~old in 1938; no additions have been made to the residence since that date. Deponent makes this affidavit to induce the Building Departmmnt of the Town of Southold to issue a Certificate of Pre-Existing Use for said premises. Sworn to before me this ~/ day of August ~19 84 .
