This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. GREEN, being duly sworn, deposes and says that: I make this affidavit in support of the application of MURIEL GOLD and myself, as tenants in commo~ for a Certificate of Occupancy for the property on New Suffolk Ave., Mattituck, located on Deep Hole Creek, east of Maratooka Road. This property and the house which stands on it was acquired by Lynn E. Green and m~aelf from Theron O. Worth, by deed dated January 17, 1949. It was owned by Lynn E. Green and myself as tenants by the entirety from that date until February 12, 1954, when as a result of our divorce, we became owners as tenants in common. Lynn E. Green continued to occupy the premises as her residence until her death in December 1974. Her will probated in Suffolk County, devised her undivided interest in the property tO her sister, Muriel Gold, so that the tenancy in common is now held by Muriel Gold and myself. At nv time since prior to April 1957 has this property been in the same ownership as any adjacent property. From February of 1954 on it has been a tenancy in common, first of Lynn E. Green and myself and now of Muriel Gold and myself. Prior to the purchase of the property by Lynn E. Green and myself in 1949, it was owned by Theron O. Worth, who purchased it from Philip R. Tuthill and Helen W. Tuthill in December of 1942. The property has at all the times herein referred to been listed as a separate item on the tax rolls of the Town of Southold. Attached hereto is a copy of the survey dated May 18, 1953 by Otto W. Van Tuyl & Sons on which the subject property is indicated by arrows which I have marked in ink. The only differen in the property now and in 1953,when the survey was made, is that the garage which is shown on the survey at the northwesterly corner of the property is no longer in existence. The house remains as it was. WHEREFORE, of Occupancy be issued. I respectfully request that a Certificate Sworn to before me this lat day of March 1976. - 2 - HOUSING CODE INSPECTION March 4, 1976 5550 New Suffolk Ave. R-1 Mattituck, N. Y. Tax Roil: Richard & Lynn Green Unoccupied Upon receipt of an application for a Pre-Existing Certificate of Occupancy, I made an inspection of this one story framed dwelling. I picked up the keys from Nina de Mo~, Four Sails Realty office, Main Road, Cutchogue, New York, and began inspection at approximately 10:05 A.M. The building consists of two bedrooms, a full bathroom and a large L shaped room containing kitchen facilities and general living area. An enclosed porch is located on north side of building for almost entire length of building. An enclosed porch provides east entry to building. Heat is furnished to building from a gas fired warm air floor furnace approximately centered in building with heat outlet discharging into living room area. The following violations of The Housing Code, Chapter 52, Town of Southold, were found: Front ~orch - constructed on locust posts, not tightly skirted, joists of porch floor lying in soil (partially excavated). Article III, Section 52-30 C - Article VI, Section 52- 63 B. Cellar (Partial basement, cement block foundation). East wall, mortar joints open at seventh and eighth course, approximately 3/4" open, wall slightly pushed in from ground pressure. Article III, Section 52- 30 A.B. Gas fired hot water heater and gas fired warm air furnace, no shutoff valves at units, Article V, Section 52-52 B.2. No water supply system (electric pump stolen, not replaced as of this inspection) Article V, Sec. 52-51 B (1.). Housing Code Inspection -2- March 4, 1976 Richard & Lynn Green 5550 New Suffolk Avenue Mattituck, N. Y. Porch Kitchen - east side, wooden platform at entry, uneven. Article III, Section 52-30 B. gas cooking range, no shutoff valve at unit. Article V, Section 52-52 B.2- Doorway between east porch and kitchen - glass pane fell out, pane broken and taped together and taped in opening of door;(replaced glass pane in opening of door). Article II, Section 52-27 A. Bedroom northeast, one window on east to provide natural light and ventilation. Under- sized;two windows on north wall open in to-enclosed front porch. Article II, Section 52-24 C (1) (3). Slab in front yard area extends beyond property line into public highway right of way. A violation of Chapter 100 Zoning Ordinance, Town of Southold, Article III, Section 100-30 A.2.d. Inspection completed at approximately 10:35 A.M. Key was returned to Nina de Mo~ at realty office.
