That I reside at 157 Summit Way, Roanoke Virginia. 2. That I am one of the present owners of the property known as 641 Kerwin Boulevard, Greenport, New York and am very familiar with the structures located at said address, as shown on the survey dated October 13, 1987, (copy annexed here). My father and mother owned said land for many years prior to 1957 and constructed the present home located thereon in the year 1954. 3. All of the structures shown on said survey were fully completed before 1957, and have remained in their current state without expansion from said date, except for an addition completed several years after 1957. This construction was done by Bauer Builders and I believe that a certificate of occupancy was obtained at that time for my father and mother, Joseph H. and Grace E. Gogerty, but that this document has been lost. 4. I make this affidavit knowing full well that the Town of Southold Building and Zoning Department will rely upon the facts as stated herein to issue a pre-existing Certificate of Occupancy for said structure and a duplicate Certi icate of Occupancy for the addition constructe after zo 'ng.
