APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:

Upon receipt of an application for a Pre -Existing Certificate
Of Occupancy, I made an inspection of the buildings on this premises.
I admitted myself to dwelling with key provided by the Matt -Agency,
Main Road, Mattituck, New York and began this inspection at approx-
imately 11:25 A.M.
Dwelling is a two story, wood framed structure with stucco siding
constructed on a poured concrete foundation. First floor has a front
entry hall, a two sectioned living room, dining room, kitchen and
half bathroom off hallway connecting kitchen and dining room. Second
floor has tree bedrooms and full bathroom. Heat is furnished to all
rooms from an oil fired steam heat furnace located in partial cellar.
A roofed over porch is on north side of building and continues around
part of the westerly side. An enclosed:eafe�i unheated porch is on
south west corner of building providing a rear entry to kitchen. A
wood framed two car garage is located in rear yard area.
The following violations of the Housing Code, Chapter 52, Code of
the Town of Southold were found:
Porch stens - at front entry to dwelling, bottom step is deteriorating
tread of stair is spreading causing excessive openings in step. j
Article III, Section 52-30 A.
Front Entry --Ceiling fixture in hallway not working, it appears
that this fixture serves to light stair for ascent & descent
since wall switches exist at first and second floor landings.
Article II, Section 52-26 B.3 -Article V, Section 52-56 B.2.
Kitchen -porcelain electric light fixture mounted on trim of
window over.sink. Wire supplying current to fixture has two
male plugs, one is plugged into electric wall outlets behind
range and plug at other end of lamp cord is plugged into outlet
on light fixture - Article V, Section 52-56 A.1.
Interior access to cellar -door from kitchen swings into cellar
stairwell -saddle of doorway is raised the same as riseesof cellar
stair. The top landing of cellar stairway is spli , i.e. saddle
of doorway continues into landing and forms a tread within
swing of door. Article II, Section 52-27 A. wINoArL
Electrical -Bedroom north west on second floor -wall fixture.
Exposed wire running from escutcheon_ to light socket. Insulation
is dried out and cracked. Article V, Section 52-56 A.1.

Bathroom -second floor. Floor tale at base of toilet and
pedestal for sink is curled and open at joints of tiles.
Atticle III, Section 52-32 D.
Electrical -cellar -open electric junction box on first floor
jfist over furnace. Article V, Section 52-56 A.1.
Furnace -smoke stack connection to chimney is open, seal has
broken away and smoke pipe is insufficiently indexed into
chimney flue. Article V, Section 52-53 B.

Stairway -Exterior entry to cellar, top s of stair is
missing. Article II, Section 52-28 B.
Exterior -soffit of cornice on east side of building has a section
that is rotted and deteriorated. Article III, Section 52-30 C.
Accessory -two car garage -trim on cornice on east side of
building has a large hole and it appears wood has been worn
away by small wild life. Article III, Section 52-30 C.
Inspection was completed at approximately 12:00 P.M., key was
returned to the Matt Agency.
Ceiling light in the cellar was found to be on when inspection
was made, light was left on accordingly.
