APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
C. Fees:

2. Certificate of occupancy on pre-existing dwelling or land use $5.00
3. Copy of certificate of occupancy $1.00

James F. Gildersleeve, being duly sworn, deposes and
says:
1. I make this affidavit in connection with my application for
a Certificate of Pre -Existing Use covering premises on. the
northerly line of Pike Street (the "Property") beginning 100.0
feet easterly from Love Lane and more particularly described on
Schedule A attached hereto.
2. The Property was acquired by me and my wife by deed dated
August 12, 1959. Prior to that date it had been owned by my
father James A. Gildersleeve and by my uncle, Sidney Gilder-
sleeve for many years and I have been familiar with the use and
operation. of the Property since my childhood.
3. Many years prior to 1957 the Property was a portion of a
larger parcel of land immediately to the west used for the
operation. of a retail store. A garage building was located on.
the Property and it was used to house delivery vehicles and to
store merchandise. After the sale of the parcel to the west,
the garage building continued to be used for storage and for
housing commercial vehicles for many years prior, during and
subsequent to 1957. Upon my acquisition of the Property in.
1959 I continued to rent it for such commercial purposes.
Thus the Property has been in continuous use for commercial
purposes prior to April 23, 1957 and continues to be used for
such purposes to the present time.

All that certain plot, piece or parcel of land with
the buildings and improvements thereon erected, situate,
lying and being at Mattituck, intthe Town of Southo34, County
of Suffolk and State of New York, bounded and described as
follows:
BEGINNING at a monument on the northerly line of
Pike Street, 100.0 feet easterly along said northerly line
from Love Lane, said point of beginning being the southeast-
erly corner of land now or formerly of Schatt; from said point
of beginning running along said land now or formerly of Schatt,
north 32 degrees 00 minutes west 132.9 feet, more or less,
to land now or formerly of Long Island Railroad; thence along
said land now or formerly of Long Island Railroad, north
45 degrees 41 minutes 10 seconds east 89 feet, more or less,
to land now or formerly of Fischer; thence alon said land
now or formerly of Fischer, south 26 degrees 43 minutes
10 seconds east 159 feet, more or less, to said northerly
line of Pike Street; thence along said northerly line of
Pike Street, south 63 degrees 08 minutes 30 seconds west
72.6 feet, more or less to the point of beginning.
LESS, however, so much of the above described plot,
piece or parcel of land alleged to be established as a public
highway (or as a portion of a public highway) by the Super-
intendent of Highways of the Town of Southold, Suffolk County,
New York, by Order dated August 27, 1974.
