APPLICATION FOR. CERTIFICATE OF OCCUPANCY
A. This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.'
B. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and
"pre-existing" land uses:
1. -Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and'a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Upon receipt of a request for a Pre -Existing Certificate of
Occupancy from Gary F. Olsen, Attorney, Mattituck. I made
an inspection of the buildings on this property. I picked
up key from Wickham & Lark, P.C., Mattituck, and began in-
spection at approximately 9:50 A.M.
The prime building is a one story framed dwelling. There
are two bedrooms, living room, dining room, full bathroom,
kitchen and laundry room. A screened porch at the rear of
building runs approximately the entire length of the living
room and encloses exterior entry to living room. Heat is
furnished to all rooms from an oil fired warm air furnace
located in partial full cellar. Crawl space of building
foundation is under laundry room and addition to north bedroom.
These areas are accessible from full cellar.
An accessory building is located in rear yard along
south side of property line. A former garage, this building
has been altered and the interior has been partitioned
(framing only) into four rooms and bathroom. A sewage
drainage system has been installed in slab of foundation.
(An informal discussion with the Board of Appeals and present
owner was held sometime in 1969 with regard to making living
unit in this building. The Board's recommendations were that
living roomfwould be permissible but kitchen facilities would
not be permitted.)
The following violations of The Housing Code, Chapter 52,
Town of Southold, were found:
Dwelling:
Entrances, front and rear, no means of controlling
light in room on entry. Article V, Sec.
52-56 B.2.
Kitchen Gas cooking range, no shutoff valve at
unit. Article V, Section 52-52 B.2.
Bathroom water supply line to water closet, leak
at elbow. Article V, Sec. 52-51 A.2.

Cellar: Waste drains from kitchen sink and laundry
washing machine run through foundation
walls and would appear to run directly
to disposal system. No connection to
building soil line system within building.
Article V, Sec. 52-51 C.1 - water supply
pump motor, two were pigtail to wall outlet.
Article V, Sec. 52-56 A 1-5. Hot water
heater, electric, repair made to tank at
terminal block and thermostat control,
structural defect. Article V, Sec. 52-50 A.
Cellar Stairway - newel damaged by termites. Art. III,
Sec. 52-30 A, Sec. 52-32 A.
Accessory Building: Several broken windows on west
side and entry door, Article III, Sec. 52-31 B.
Sill along south wall on entry level - termite
damage, Article III, Sec. 52-30 B., Sec. 52-31 A,
Sec. 52-32 A. Building sewage soil line on
exterior of foundation subject to freezing.
Article V, Sec. 52-50 B.
Inspection completed at approximately 10:30 A.M. Key was
returned to the office of Wickham & Lark, P.C.
