The validity of this permit is o~' may be subject to the approval of other governmental or municipal
authorities. The Town accepts no responsibility in app[yidg for or obtaining such approval. In the
· event that such approval is necessary~ the holder of this permit shall not cbmmence operations here-
under until such approval has been obtained in writing. The failure to obtain such other approval when
' required shall subject this permit- to immediate revocation by the. Trustees upon receipt by the
Trustees- of written notice from such other governmental or municipal authorities 0f its refusal or.
disapproval'~ - -
The applicant does by the acceptance of this permit, assume all responsibility for operations under-
taken pursuant to this permit, and shall take all precautions for the prevention of injuries to persons
and property resulting from such operations. By such acceptance, the applicant also agrees to indemni-
fy and save harmless the Town, and its officers, agents and employees from any and all claims arising
from operations under this permit and any and all acts or omissions of applicant, his agent and
employees.
The applicant and the owner and occupants of the premises u pon which the operations authorized by
this oermit are being conducted, do, bY the acceptance of this oermit, give consent to the Town, and
its officers an(] employees to enter uoon the premises where such operations are Being conducted to
make such inspections as the Town may deem necessary to insure that such operations are being con-
ductad in conformity with this permit.
Thi¢ operation will not substantially:
A. Adversely affect the wetlands of the town.
B. Cause damage from erosion, turbidity or siltation,
C. Cause saltwater intrusion rotc the fresh water resources of the town.
D. Adversely affect fish, shellfish or other beneficial marine organisms, aquatic wildlife and vege-
tation or the natural habitat thereof.
E. Increase the danger of flood and storm-tide damage.
F, Adversely affect navigation on tidal waters or the tidal flow of the tidaI waters of the town.
G. Change the course of any channel or the natural movement or flow of any waters.
H. Weaken or undermine the lateral support of other lands in the vicinity. -
I. Otherwise adversew affect the health, safety a,nd general welfare of the people of the town.

Your request has been approved by resolution, adopted at this

The following action was taken by the Board of Town Trustees during
their regular meeting held on July 27, 1987 regarding the above matter.
WHEREAS, John C. W. Campbell applied to the Southold Town Trustees for a permit
under the provisions of the Wetland--Ordinance of the Town of
Southold, application dated May 5, 1987, and
WHEREAS said application was referred to the Southold Town
Conservation Advisory Council for their findings and
recommendations, and
WHEREAS, a public hearing was held by the Town Trustees with
respect to said application on July 27, 1987 au which time ali interested
persons were given an opportunity to be heard, and no objections were raised, and
WHEREAS, the Board members have personally viewed and are
familiar with the premises m question and the surrounding area, and
WHEREAS, the Board has considered all the testimony and documentation
submitted concerning this application, and
WHEREAS, the Board has determined that the project as proposed will
not affect the health, safety and general welfare of the people of
the town,
NOW, THEREFORE, BE IT
RESOLVED that John C. W. Campbell BE AND HEREBY IS GRANTED PERMISSION
UNDER THE
PROVISIONS OF THE WETLAND ORDINANCE OF THE TOWN OF SOUTHOLD TO:
Construct a single family residence on property located on Harbor Lights Drive,
Southold. This permit is approved with the provision that dry wells are installed to
contain run off and there is to be no fertilization used on the lawn.

This permit will expire on July 27, 1989 if work has not commenced by
said date. There are two inspections required and the Trustees are to
be notified upon the completion of the work. Prior to the issuance of
said permit by the Clerk, the applicant shall file with the Clerk a certificate
that he has public liability insurance policies insuring against any liability
which may arise in the performance of the operations pursuant to such permit
in such amount as shall be fixed by the Trustees, which said policies shall
name the Town as a name insured. Please take further notice that this
declaration should not be considered a determination made for any other
department or agency which may also have an application pending for the
same or similar project.
Please remit $10.00 for Wetland Inspection Fees at this time.

The Southold Town Conservation Advisory Council held its regular meeting
June 18, 1987. The following recommendations were made at that meeting:
No. 527 - Mr. and Mrs. John Lang - Recommend approval with the stipula-
tion that applicant does not distub the natural vegetation (spartina
wetland).
No. 528 -_Mr. and Mrs. Alan Watson - Recommend approval.
No. 529 - Hide-A-Way Boat Basin- Recommend approval.
No. 530 - West Lake Association - Recommend approval.
No. 531 - Alexander Koke - Recommend approval of the berm and filling.
No. 532-

Gardiners Bay Property Owners Association - Recommend approval.
Yan H. Rieger - Recommend approval.
No. 534-
Peter Fakiris - Recommend approval of the dock but disapprove
of the bulkheading. The CAC suggests applicant extends the
ramp from the base of the dock up further and that would eliminate
the need for the bulkheading. The CAC does not approve of the
bulkhead because there are no other bulkheads in the area and
the bulkhead would create erosion problems.
No. 536 - John C.W. Campbell - Recommend approval with the stipulation that
a 50, natural vegetation buffer be maintained between the house and
bulkheading.
No. 510 - James E. Fitzgerald Jr., Pres., Broadwaters Cove Association, Inc.
Recommend approval.
No. 522 - Michael S. Gillilund - Recommend approval with the stipulation that
it be mandatory that applicant does not disturb the natural
vegetation buffer between the house and bluff.

General Information (Read Carefully)
- In completing the form the reviewer should be guided by the question: Have my decisions and determinations
been reasonable? The reviewer is not expected to be an expert environmental analyst.
- Identifying that an effect will be potentially large (column 2) does not mean that it is also necessarily
significant. Any large effect must be evaluated in PART 3 to determine significance. By identifying an
effect in colunm 2 simply asks that it be looked at further.
