Box 1179 Southold, New York 11971 October 7, 1991 En-Consultants 1329 North Sea Road Southampton, NY 11968 Re: Pat Colagiuri SCTM ~1000-123-3-23 & 24 Dear Mr. Haje: The following action was taken by the Board of Town Trustees during its regular meeting held on October 3, 1991 regarding the above matter: WHEREAS, ROY HAJE ON BEHALF OF Pat Colagiuri applied to the Southold Town Trustees for a permit under the provisions of the Wetland Ordinance of the Town of Southold, application dated July 24, 1991 and, WHEREAS said application was referred to the Southold Town Conservation Advisory Council for their findings and recommendations, and, WHEREAS, a public hearing was held by the Town Trustees with respect to said application on October 3, 1991 at which time all interested persons were given an opportunity to be heard, and, WHEREAS, the Board members have personally viewed and are familiar with the premises in question and the surrounding area, and, WHEREAS, the Board has considered all the testimony and documentation submitted concerning this application, and, WHEREAS, the Board has determined that the project as proposed will not affect the health, safety and general welfare of the people of the town, NOW, THEREFORE BE IT RESOLVED THAT ROY HAJE ON BEHALF OF PAT COLAGIRUI BE AND HEREBY IS GRANTED PERMISSION UN-DER THE WETLAND ORDINANCE TO: install from an existing bulkhead, a 3' X 12' hinged ramp leading to a 6' X 30' float. Float will be secured by (2) sets of 2 pile dolphins. This permit will expire two years from the date it is signed. Fees must be paid, if applicable, and permit issued within six months of the date of this notification. Two inspections are required and the Trustees are to be notified upon completion of said project. Please return to the Building Department for a determination on the need for any other permits which may be required for this project. Southold Town Board of Trustees SouthoId Town Conservation Advisory Council October 2, 1991 Recommendation of Wetland Applications On - 3 1991 TOWN OF SOUTHOLD The following recommendations were made by the CAC at its meeting held Thursday, September 26, 1991: On a motion by John Holzapfel, seconded by Bruce Loucka, it was RESOVLED to recommend to the Southold Town Trustees approval of Wetland Application No. 35-7-4 of Pauline Israel to construct a 4' x 12' catwalk, 3' x 10' ramp and 6' x 20' float. The CAC suggests everyone move to their own proper space. 12 Osprey Nest Road, Greenport Vote of Council: Ayes: All Motion carried. The permit sign enclosed with the permit or a copy of letter of approval must be protected from the weather and posted in a conspicuous location at the work site until completion of authorized Work. 3. At least 48 hours prior to commencement of the project, the permittee must complete and return the top portion of the enclosed receipt form Certifying that he is fully aware of and understands all provisions and conditions of this permit. Within one week of completion of the permitted work, the bottom portion of that form must also be completed and returned, 4. If the pevmittee intends to undertake any project design modifications after permit issuance, the permittee must submit the appropriate plan changes for approval by the Regional Permit Administrator prior to undertaking any such modifications. The permittee is advised that substantial modification may require submission of a new application for permit. 5. Those portions of docks, catwalks and ramps which extend over vegetated tidal wetlands shall have a maximum surface width of ~ feet and shall be a minimum of 3 1/2 feet above grade, as measured from the ground surface to the bottom of the planking. Floating docks shall not rest on vegetated tidal wetlands at any tidal stage. FOR THE DISTRICT ENGINEER: ~ief, Regulatory Branch SUPPLEMENTARY SPECIAL CONDITIONS The A. following conditions apply to all permits: If any of the permit conditions are unclear,the permittee shall con- tact the Division of Regulatory Affairs at the address and telephone noted below. A copy of this permit with supplementary conditions and approved plans shall be available at the project site whenever authorized work is in progress. The permit sign enclosed with the permit or a copy of letter of approval shall be protected from the weather and posted in a con- spicuous location at the work site until completion of authorized work° At least 48 hours prior to commencement of the project, the permittee shall complete and return the top portion of the enclosed receipt form certifying that he is fully aware of and understands all provisions and conditions of this permit. Within one week of completion of the permitted work, the bottom portion of that form shall also be com- pleted and returned. For projects involving activities to be accomplished over a period of more than one year, the permittee shall notify the Regional Permit Administrator in writing at least 48 hours prior to the commencement of resumption of work each year. If project design modifications take place after permit issuance, the petmittee shall submit the appropriate plan changes for approval by the Regional Permit Administrator prior to undertaking any such modi- fications. The permittee is advised that substantial modification may require submission of a new application for permit. Ail necessary precautions shall be taken to preclude contamination of any wetlands or waterway by suspended solids, ssdiment, fuels, solvents, lubricants, epoxy coatings, paints, concrete, leachate or any other environmentally deleterious materials associated with the project work. Any failure to comply precisely with all of the terms and conditions of this permit, unless authorized in writing, shall be treated as a violation of the Environmental Conservation Law. The permittee is advised to obtain any permits or approvals that may be required from the U.S. Department of Army Corps of Engineers, NY District, 26 Federal Plaza, New York, NY 10278, (Attention: Regulatory Functions Branch), prior to commencement of work authorized herein. The granting of this permit does not relieve the permittee of the responsibility of obtaining a grant, easement, or other necessary approval from the Division of Land Utilization, Off£ce of General Services, Tower Building, Empire State Plaza, Albany, NY 12242, which may be required for any encroachment upon State-owned lands underwater.
