It is my/our understanding that we are to maintain this
structure with materials designed to endure 30 years and or
equivalent to the original approved structure and that for any
maintenance which involves more than 5% in length of this
structure we are to give prior written notice to the
administrator, waiting for their approval in all but true
emergency situation's which would seriously endanger life,
property and important coastal natural resource features such as
bluffs or wetlands. We further understand that failure to
maintain the structure could result in a requirement to post a
bond and or have the repairs ordered as a lien against the
proper~y upon a finding by the Trustees that said lack of
maintenance would risk life, property or important natural
resources features including protected, rare or threatened
animals and plants.

SPECIAL CONDITIONS
During construction, concrete orleachate shal!not escape
or be discharged,.nor shallwashings from transit mix
trucks, mixers, or other devices enter tidal Wetlands and or
protected buffer areas.
Any debris or excess material from construction of this
project shall becompletely removed from the adjacent area
(~pland) and removed to an approved upland area for
disposal. No debris is permitted in tidal wetlands and or
protected buffer areas.
Ail fill shall consist of "clean"gravel and soil (not
asphalt,'slag~ fiyash, broken concrete or.demolition
debris).
~o excavation of the beach is authorized for the purpose of
obtaining fill or stone materials.
All peripheral berms, Cofferdams, rock revetments, seawalls,
gabions, bulkheads etc. shall be completed prior to
placement of any fill material behind such structures.
There shall be no discharge of runoff or other effluent over
or through any bulkhead or shoreline stabilization structure
or into any tidal wetland or adjacent area.

A. A copy of this permit, including all condinlons and
approved plans, shall be available au the pro]ecu site
w~never authorized work is in progress. The permit
sign ~nclosed with the permit shall be prouected from
the weather and posted in a conspicuous location at the
work site until all authorized work has been completed.
B. The permittee shall require nhat any contractor,
projec= engineer, or other person responsible for the
overall supervision of this projec~ reads, understands,
and complies with this per, it and all its general,
special, and supplementary special conditions. Any
failure =o comply precisely with all of the terms and
conditions of this permit, u~!ess authorized in
%uriting, shall be treated as a violation of the
Environmental ConseI"¢ation Law. If any of the permit
conditions are unclear, the per~ittee shall contac= the
Division of Regulatory Affairs at t_ke address on page
one or telephone (516) 444-0365.
C. If pro~ec= design modifications become necessary after
pe_~mit issuance, tl~e per~ittee shall submit the
approuriate plan changes for approval by the Regional
pe_~mi% Administrauor prior to undertaking any such
modifications. The per~it~ee is advised t~ha~
substantial modifica=ion may require submission of a
new application for permit.
D. A~ leas= 48 hours prior to co~encemen= of the pro,eot,

top portion of the enclosed notification form
