I hersby apply for a Permit to red!ace the'fill on
our. residential property located in Mattituck~ N.Y., on
Peconic Bay Blvd. This need for the replacement'of the fill
~ue to the erosion of approximately ?0 to ?5 yards of

Also attached are copies of four photographs taken in
early January~ 1993~ showing the extent of the erosion due
to the December storm..
Attached also are two sketches, with approximate
dimensions~ shewing the various specific areas of the storm
erosion, and the proposed replacement of the eroded fill.
The amount to be replace~ will be the same amount as that
lost during the storm, thus restoring the property to it~
original condition and configuration as before the damage
and erosion caused by the storm of December 11 and
19~. The work will De performed Oy Ray Nine Sand and
Gravel.
I have also filed, this date~ with the NYS Dept. of
Environmental-Conservation, for approval to have this storm
damage and erosion repaireO and reolaced.
I trust that this information is sufficient for the
issuance of the required permit to get this
