This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey - of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Tax Roll: Michael Fronimakis & others Unoccupied Upon receipt of an application for a Pre -Existing Certificate of Occupancy, I made an inspection of this partial two-story and one-story, wood framed dwelling. Mr. James Bitses arrived on premises at approximately 9:20 A.M. There was no key available and entry to building was made through unlocked window on north- east corner of building. The first floor has two bedrooms, living room, front entry hall, dining room, kitchen with eating area, and a room on south- east corner of building with water and waste pipes capped off in north wall. Second floor has three bedrooms and a half bathroom at west end of hallway. A partial cellar accessible by exterior stairway at rear of one-story is constructed of red brick and mortar. The balance of building's foundation had no means for inspection and therefore structural determination was not possible. There is a wood framed 6' by 8' storage building in rear yard close to dwelling. At north end of property, remains of a barn and a small wood framed structure are collapsing on their foundations. The following violations of the Housing Code, Chapter 52, Town of Southold, New York, were found: Front entrr - no means of controlling..light in hall at point of entry, Article V, Section 52-56 B 2. Kitchen - gas cooking range, no shut off valve at unit, Article V, Section 52-52 B 2. Cooking range is located in corner of room and side of unit is in contact with sheet rocked wall, Article V, Section 52-50 A. Hallway leading to bedrooms on north side, ceiling light fixture removed, light socket pig tailed, exposed twisted wire connection, Article V, Section 52-56 A 1. Half bathroom - second floor, no window or mechanical ventilation. Ceiling light fixture removed, bare wires exposed, Article II, Section 52-26 B 4(a.b.) and Article V, Section 52-56 A 1. SAS N max Cellar - nogcellar window, loose sheet of plywood laying lomat on stairwell for cellar stair. Ceiling light fixture is pigtailed light socket, twisted connection and exposed, Article III, Section 52-31 A. and Article V, Section 52-56 A 1.
