Instructions .
A. This application must be filled in typewriter OR ink, and submitted-ieetato the Building Inspec-
torwith the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features. -
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal}.
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
- lions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957, Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Oate of any housing code or safety inspection of buildings or premises, or other pertinent informs-

GAIL CONSTANTINE FOY, also known as GAIL CONSTANTINE, being duly
sworn, deposes and says:
1. I am one of the owners of the premises located at 1680 Smith
Road, Peconic, New York 11958 and known on the Tax Map for the
County of Suffolk as District 1000, Section 98, Block 3, Lot 21.
2. My father and mother purchased this property on September 26,
1953 and I inherited part ownership of the house upon their death.
Located on the premises are a one-family house and
garage.
I hereby request the issuance of a pre-existing Certificate
of Occupancy and I hereby state that the structures on the said
premises were all erected prior to April 23, 1957. I further
state that the dwelling on the premises has been used as a dwell-
ing since that time with no changes and that same is in habitable
condition.
