IT IS FURTHER CERTIFIED that, based upon information presented to
the Building Inspector's Office, the occupancy and use for which this
Certificate is issued is as f ollows: Property contains a 2 story 2 family wood
framed dwelling, twe car garage and a wood framed uti ity ui ding. All situated in
'A' Zone with access to Lipco Road and Sound Avenue. Building Permit /P14781Z

Instructions
A. This application must be filled in typewriter OR ink, and submitted ~i~~ to the Building Inspec-
torwith the fo{lowing; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supp{y and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, acertificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

ALICE GRATTAN DUFFY, being duly sworn, deposes and says:
1. I am the previous owner of the property located on
the West side of Lipco Road, Mattituck, New York, as shown on the
survey of R. Van Tuyl dated June 14, 1983, a copy of which is
annexed hereto, and described on the Suffolk County Tax Map as
1000-141-3-17.
2.. I have been familiar with the property and the three
structures located thereon since the 1940's, and have had
discussions with neighbors and others concerning the construction
and use of these three buildings.
3. All of the structures shown on said survey were fully
completed prior to April 9, 1957, and have remained in their
current state without expansion from said date.
4. I make this affidavit knowing full well that the Town
of Southold Building and Zoning Department will rely upon the
facts as stated herein to issue a certificate of pre-existing use
for the two story frame house as a two family dwelling, the garage
and the frame building located thereon.
