Instructions
A. This application must be filled in typewriter OR ink, and submitted ~ to the Building Inspec-
torwith the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Undervvriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
lions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

MILDRED KNIPPER being duly sworn deposes and says:
I reside at 57 Dartmouth Street, Garden City, New York and
am the executor of the estate of my late sister, JOSEPHINE M.
FLACK, who died on September 19, 1987.
Among the decedent's assets, was a single family home
located at Huntington Boulevard, Town of Southold, County of
Suffolk known as Section 067.00, Block 03.00, Lot 008.00. I am
fully familiar with the property having spent a number of summer
vacations there with my sister and other family members.
The property was purchased by my sister from a Helen Fulton
by a deed dated June 7, 1955. The property was used as a summer
house by my sister and family members each year from 1955 through
approximately 1986, when my sister became ill.
I have reviewed the new survey of the property prepared by
Bohn & Bonacci, P.C. dated 4/23/88 as well as the appraisal of said
property prepared by John P. O'Rourke & Associates Inc. and the
pictures contained in said appraisal report. Both the survey and
the pictures taken from the appraisal report accurately reflect all
the improvements which were on the property when my sister purchased
it in 1955. She never made any structural additions or changes
other than painting and normal maintenance throughout the years.
With respect to the wood frame shed, which is approximately
two feet from Dickinson Street, I hereby represent to the Town
of Southold and on behalf of the estate this wood frame shed will
be removed.
The property is currently under Contract of Sale, but in
order to sell, the estate must deliver a Certificate of Occupancy
for the subject premises. I therefore make this affidavit in
order to induce the Building Department of the Town of Southold to
issue a Certificate of Occupancy for the subject premises as re-
flected on the survey of Bohn & Bonacci, P.C. dated 4/23/88.
