A. This application must be filled in typewriter OR into, and submitted in duplicate to the Building lnsp
for with the following; for new buildings or new use:
1. Finat survey of property with accurate location of all buildings, property lines, streets, and unus<
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S•9 form or equal). •
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial twildings, Industrial buildings, Multiple Residences and similar puddings and instal
bons, a certificate of Code compliance from the Architect or Engineer responsible for the buildir
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existin

1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural

tion in the Southold Town Building Inspectors Office for the following:

That the undersigned do(tes) hereby give consent to the Building Inspectors
of the Town of Southold to enter upon the above described property, including
any and all buildings located thereon, to conduct such inspections as they may
deem necessary with respect to the aforesaid application, including inspections
to determine that said premises comply with all of the laws, ordinances, rules
and regulations of the Town of Southold.
The undersigned, in consenting to such inspections, do(g) so with the
knowledge and understanding that any information obtained in the conduct of
such inspections may be used as evidence in subsequent prosecutions for vio-

WILLIAM FIORE and GRACE FIORE, being duly sworn, deposes
and says:
That we are the owners of a parcel of land on Bayberry Road,
Southold, New York (Suffolk County Tax Map No. District 1000,
Section 76, Block 1, Lot 8}. Lot #17 on the "Map of West Creek
Development"was acquired on June 1, 1940 from Georqe M. Tisdale
by William Fiore (Liber 2113 Page 475) and Lot #18 on the "Map of
West Creek Development" was acuired on July 14, 1943 from Joseph
A. Krupski by William Fiore and Grace Fiore (Liber 2322 Page 565).
` Located on the property is a one-story frame house an a
detached garage which has been used continuously as a residence
since 1940. Also located on the premises is a barn. All of the
buildings are in good condition and have been constructed prior
jto 1957.
~I The application is made for the purpose of obtaining a
pre-existing Certificate of Occupancy from the Building Department
of the Town of Southold.
