APPLICATION FOR CERTIFICATE OI' OCCUPANCY
This application must be filled in by typewriter OP, ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water. supply and sewerage-disposal (S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2J10 of 17 lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:
1. .lc curate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
2. A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

Alterations to dwelling $25.00, Swimming pool $25.00, Accessory building $25.00,
Additions to accessory building $25.00. Businesses $50.00.

Copy of Certificate of Occupancy - $5.00 over 5 years - $10.00

3. That Stephen Figall seeks to obtain a Certificate
of Preezisting Occupancy (Pre-C.O.) and represents that he
acquired title to the land on May 22. 1967 by deed being
recorded at the Suffolk County Clerk's Office at Liber 6160
page 59 on June 1, 19671
4. That title was acquired Eros Albert D. Dawson and
Dorothy M. Dawson, who in turn acquired the parcel, referred
to as Parcel III in Deed dated August 23, 1949 recorded in
the Suffolk County Clerk's Office at Liber 3130 page 427 fro>a
Robert Haasond McLeods
5. That the records of the Town Assessor show that
starting in 1949, the parcel was assessed as an iYproved
parcel with a one-story Era's dwelling;
6. That the dwelling continues to ezist without any
structural alterations to thi• date.
WHEREFORE, it ie respectfully requested that a
Certificate of (Preezisting) Occupancy be issued for the
subject dwelling.
