This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(5-9 form or equal). 3. Approval of electrical installation from Boo rd of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and .similar buildings and installations, o certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn sfiatement of owner or previous owner as to use, occupancy and condition of buildings. 3. Tax Roll: Robert A. Dow & wf. occupied: owner Upon receipt of an application for a Pre-Existing Certificate of Occupancy I made an inspection of the buildings on this premises. Accompanied by Curtis Horton, we were admitted to dwelling by Mrs. Dow and began this inspection at approximately 3:05 P.M. The dwelling is a two story wood framed structure constructed on a red brick masonry foundation. First floor contains a utility room that provides rear entry to building. A kitchen, dining room, living room, den with fireplace, two bedrooms, full bathroom and a front entry hallway. Second floor is accessible by prime stairway from front entry hallway and a secondary stairway. There are three bedrooms and a full bathroom on second floor. Heat is furnished to all rooms from an oil fired hotwater furnace located in partial cellar. A wood framed three room accessory structure with an attached work shop and garage is located in rear yard. This structure is constructed on a red brick masonry foundation with a full cellar accessible from work shop area. A detached carport is located behind this structure. The following violations of the Housing Code, Chapter 52, Code of the Town of Southold, N.Y, were found: Dwellin~- Entr -rear, to utility room, no means of controlling light in room at point of entry. Article V, Section 52-56 B.2. Gas Cookin ran e-no shut off valve at unit, Article V, Section - B. . Electrical-secondary stair to rear hallway of second floor, e ec ric fight wall fixture, light socket is supported by wires supplying electric current to fixture. Porcelain electric light fixture in ceiling in hallway is not securely fastened. Bedroom on south east side of second floor, light fixture re- moved from east wall and exposed wires protrude from electrical box unprotected. Shut off throw switch box for oil fired furnace in cellar is not securely fastened. Article V, Section 52-56 A.1 & 2. Railin -for stairwell on second floor is excessively shakey and ne~post is not firmly fastened. Article 222, Section 52-30 A. Accessory structure- Roof-leaks in work shop area.
