A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic feature[
2. Final al~proval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to I~repare a certificate.

JOHN DI VELLO, residing at (No #) Westphalia Road,
Mattituck, New York 11952, being duly sworn, deposes and
says:
I am the owner of premises, located on Westphalia Road,
Mattituck, New York, Suffolk County Tax Map #1000-113.00-
09.00-008.000 & 015.000, as shown on the enclosed survey,
and shown as Lot #4 on Minor Subdivision Map, also enclosed.
Prior to 1900 this property was owned by Meday and it
is believed that the residence on it was erected prior to
that time. The E. Belcher Hyde Map, 1909 shows the premises
as owned by Meday and shows a residence located on it. The
residence is in its original condition.
The premises and more were purchased by Mrs. DiVello
and myself on October 24, 1978 from Freda S. Rau & Ernest E.
Schutt and to my knowledge no changes or alterations have
been made to the premises that would require a building
permit or certificate of occupancy, since 1957.
I make this application to the Town of Southold for a
Certificate of Pre-Existing Use for the residence.
