Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"_
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topograph ic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings·
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Sound AVenue, Soutlx)ld , which is shown and designated on the Suffolk
County tax map as District 1000, Section 135 , Block 02 , Lot
That the undersigned (has) (have) filed· or caused to be filed, an applica-
tion in the Southold Town Building Inspector's Office for the following:
Certificate of pre-existing use for a two-family residence
That the undersigned do(es) hereby give consent to the Building Inspectors
of the Town of Southold to enter upon the above described property, including
any and all buildings located thereon, to conduct such inspections as they may
deem necessary with respect to the aforesaid application, including inspections
to determine that said premises comply with all of the laws, ordinances, rules
and regulations of the Town of Southold.
The undersigned, in consenting to such inspections, do(es) so with the
knowledge and understanding that any information obtained in the conduct of
such inspections may be used as evidence in subsequent prosecutions for vio-
lations of the laws, ordinances, rules or regulations of the Town of Southold.

MARGARET DELLANNO, being duly sworn, deposes and says:
1. That she resides at 51255 County Road 48 at the corner
of Clark Road and Sound Avenue in the Hamlet and Town of Southold
and she makes this affidavit in order to obtain a Certificate of
Occupancy or Pre-Existing Use for a two-family residence.
2. That your deponent has resided at the subject residence
since January, 1952, on a continuous basis. For many years, the
premises were a tourist home and then a three-family dwelling and
then a two-family dwelling. In July~ 1982, the premises were determined
by the Zoning Board of Appeals of the Town of Southold to be a legal
two-family dwelling.
3. That the premises continue to be a two-family dwelling
and the use of same has not been discontinued or abandoned at any
time.
4. That the subject premises are maintained in first
class condition and your deponent has utilized the best craftsmen,
including carpenters, plumbers, electricians, and the like, in this
community for many years to continually and at substantial expense
upgrade and maintain the facilities at the property.
5. That the premises are inhabited at the present time
and completely habitable. That your deponent is aware of the
limitations within the Zoning Board of Appeals determination of
August 12, 1983, and has advised the purchaser of the subject premises
of the same limitations by including and making as part of the contract
of sale of the subject premises the two page decision of the Zoning
Board of Appeals under Appeal No. 2998.
