APPLIGATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink,, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, end
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage dlsposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings end
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), No~-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

Upon receipt of an application for a Pre-Existing Certificate
of Occupancy I made an inspection of the buildings on this premises.
I was admitted to dwelling by Pete Stoutenberg who accompanied me
during this inspection that began at approximately 3:40 P.M.
The dwelling is a wood framed two story structure constructed
on a ruble stone foundation that was built into a bank in a manor
so as to provide entry for second story at grade level. First
floor contains a living room, with a fireplace, dining room with
large fireplace that contains an oven for baking. Also a kitchen
in a one story lean to attached to easterly side of building.
Second floor contains two bedrooms and a fnll bathroom accessible
through easterly bedroom. Bathroom has entry from the exterior at
grade level. Accessory structure is a wood framed one story building
constructed on wood posts. Building contains three rooms, one bed-
room with toilet in closet, a living room and kitchen.
The following violations of the Housing Code, Chapter 52,
Code of the Town of Southold, N.Y. were found:
Dwelling:
Plumbin~ facilities-no provisions for furnishing hotwater to
sinks and bathtub within dwelling. Article V, Section 52-51
Entries-kitchen, front entry hallway, no means of controlling
~n room at point of entry. Article V, Section 52-56 B.2
Kitchen-.floor, excessive deformation and settlement, does not
properly support weight applied. Article III, Section 52-30
A.B.
Chimney flue-chimney for fireplaces do not have clay flue
linings. Article V, Section 52-54 A.4.
Stairway-interior to second floor insufficient width, improper
size of landings at change of direction, insufficient tread
width and excessive rise of steps. Article II, Section 52-27
A.B.
Sewage system-no house trap or fresh air vent. Article V,
Section 52-51 C.1.

Exterior-sash and window trim deteriorating, building built
low to ground, sill and shingles deteriorating on southerly
side. Article III, Section 52-30 B.C. Section 52-31B.
Bedrooms-second floor, excessive sagging and spring of floors.
Article III, Section 52-30 A. Section 52-32 A.
Heating- no central heating system for occupancy between the
fifteenth of September and 31st day of May of the following
year. Article V, Section 52-53 A.1.
Accessory structure:
Structural-posts supporting building have moved and are leaning
in one direction and appear to be on the verge of collapse.
Article III, Section 52-30 A.B.C. Kitchen section where build-
ing is jointed, inside corner has pulled away from main
structure causing openings in wall and roof of structure.
Article III, Section 52-30 A.C., Section 52-31 B.C., Section
52-32 A.
Electrical-ceiling light fixture in kitchen, fixture rusted,
light socket of fixture hanging from wiring furnishing current
to fixture. Article V, Section 52-56 A.1.2.
Toilet room-wood floor, no moisture resistance covering, Article
II, Section 52-26.4.
Interior protection-ceiling of living room, wall boa~d lo~se
and falling sway from ceiling ~oists. Article III, Section
52-32 C.
Fire safety-smoke stack in living room, single wail'type
through ceiling and roof. Article V, Section 52-54 A.1 & B.
~o-front porch, deck and frame rotted away. Article III,
n 52-30 A.
Heatin~-no provisions for providing heat to buildings between
the fifteenth of September and the thirtY-first day of May of
the following year. Article V, Section 52-53 A.
Water supply-hand pump in room used as kitchen. Article V,
Section 52-51 B.1.2.
Plumbing facilitie~uno source of hotwater supply to plumbing
fixtures or approvea sewage system. Article V, Section 52-51D.
Inspection completed at approximately 4:20 P.M.
