Upon request of Gary Olsen, Attorney, Main Road, Mattituck,
New York for a Pre-existing Certificate of Occupancy
(application to follow), I made an inspection of this one
story framed dwelling. Violations of the Housing Code,
Chapter 52, Town of Southold, New York, are contained later
in this report.
I was admitted to the building by Mr. Ernest Willet,
a neighbor, who accompanied me during this inspection
which began at approximately 10:50 A.M.
The building's habitable area consists of two bedrooms,
one full bathroom, a combination of living room and kitchen
eat-in area. Heat is furnished to all rooms from an oil
fired hot water furnace located in full cellar. A part of
cellar serves as a one car garage with a garage door as
part of foundation wall facing the public highway, (south
side of building). An enclosed porch running the entire
length of building is on the north side (rear yard) with
entrance to porch from east side of building; this being
the only entrance to first floor. There is no inside
stairway to cellar.
Violations:
Cellar: On west wall of foundation there is a
two wire conductor with taped ends
which appears to be a hot line.
At northwest corner there is an open
junction box with a two wire (trouble
lamp type) connection running from box
along top of foundation wall and it
connects to (no junction box) conductor
leading underground to outside pump house.

Kitchen: Gas cooking range, no shut-off at unit.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
