Instructions
A. This application must be filled in typewriter OR ink, and subm~ttedeH~du!~li tl~e Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2.Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable·
&. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
lla nd uses:
· Accurate survey of peoperty showing all property lines, streets, buildings and unusua~ natural or

· Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.

AFFIDAVIT

DOROTHY R. DELANY, being duly sworn deposes and says:
I am the surviving tenant by the entirety of Fred Delany,
who died a resident of Nassau County on August 12, 1978, and I am
making this affidavit in support of ~/ application for a Certificate of
Occupancy on premises 330 Dogwood Lane, Southold, New York.
My husband, Fred Delany, and I purchased this property
as husband and wife on April 1, 1968 from Joseph and Jeanette Pokorny.
We purchased this house as a one family house, and we used and occupied
it continuously as a one family residence for ourselves. We have main-
tained the property, and I consider it to be in very good condition.
Attached hereto and made a part hereof is a survey by Otto
W. Van Tuyl and Son, dated June 22, 1964. It is a survey of my home
described above, and there have been no exterior changes since the
date of this survey. I know that neither I nor my husband Fred made
any changes since we purchased the home.
