Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2, Final approval of Health Dept. of water supply and sewerage disposel-(S-9 form or equal).'
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable,

WILLIAM WICKHAM, residing at (No #) Main Road, Cutchogue, New York,
being duly sworn, deposes and says that he is an attorney and a member
of the firm of Wickham, Wickham & Bressler, P.C. with offices at 10315
Main Road, Mattituck, New York; that he is the attorney for LOUISE
OVERTON DAY and JULIA DAY LEO, owners of the property on the West side
of Peconic Lane, Peconic, New York as shown on the attached map.
Deponent has been familiar with the property for many years
and has also discussed the same with Mrs. Day. The property was originally
owned by Mrs. Day's grandfather, JONATHAN OVERTON and then became the
property of her father, FRANK OVERTON.
The residence and garage were built many years before 1900 by
the said Jonathan Overton. Since 1957 there have been no changes in
the structures.
Deponent makes this affidavit to request the Building Department
of the Town of Southold to issue a Certificate of Pre-Existing Use
for the residence and garage.~
