This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S~9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Darnell & Wf Main House & Cottage - Occupied Upon request of the Southold Town Building Department, I made inspection of the buildings on this premises and found the following violations of Chapter 52 Housing Code, Town of Southold. First building inspected was cottage occupied by Miss Barbara Olstad, inspection begah at approximately 10:05 A.M. COTTAGE, one story framed dwelling, consists of living room, two bedrooms, kitchen and full bathroom. Heat is furnished from gas space heater located in living room. Bathroom occupies approximately one half of rear porch; remaining portion is entrance and utility room with incompleted interior. A screened front porch runs the length of the front of the building. Living Room: gas heater, no shutoff valve at unit Art. V, Sec. 52-52 B.2. Gas vent pipe runs through wallboard partition into clothes closet with rise through ceiling and terminates through roof pipe from heater to ceiling, not of the approved type for use near combustible materials. Art. V, Sec. 52-54 B. Kitchen : gas range in counter top, no shutoff valve at unit, Art. V, Sec. 52-52 B. Electric wall switch and outlet box- outlet and switches not securely fastened, no plate, Art. V, Sec. 52-56 A (1)-(2). Bathroom: Hot water heater, no pressure relie~ valve, Art. V, Sec. 52-53 I (1) - electric hookup, wires connected and taped, requires junction box, Art. V, Sec. 52-56 A.1. Water pump housed in box above ground against building subject to freezing, not completely insulated. Art. V, Sec. 52-50 B. - Two electric junction boxes require plates, (meter pump hookup). Art. MAIN HOUSE : One story dwelling, brick faced, occu- pancy by owner, consists of eat-in kitchen, dining room, living room, three bedrooms and full bathroom. Heat is furnished to each room from hot water furnace located in full cellar. An enclosed unheated porch runs the length of front of the building. Bathroom : Wall tile missing near doorway and to the right of shower door opening. Art. III, Sec. 52-30 C. Cellar: Open junction box bottom of stair- west side- three open electric junction boxes, loose switch and outlet in a gang box. To right of rear entrance to cellar- open switch box, open junction box on hot water heating unit with forced air circulation. Furnace Room: open panel box, two light fixtures not secured to boxes ~.open junction box. Hot water furnace circulat~e~, no junction box and connectAea for electrical connection - open junction box on rear wall to right of furnace; junction box on girder open with jumper running across floor from open panel box. Front end of cellar, open panel boxes all in violation of Art. V, Sec. 52-56 A. (1) - (2). Exterior: Cellar window, broken pane of glass, Art. III, Sec. 52-31. Rear dormer window - broken sash rail, two missing window panes, Art. III, Sec. 52-31 B. All trim requires painting, Art. III, Sec. 52-30C. & Sec. 52-31 B.
