A. This application must be filled in typewriter OR ink, and submitZed ,- ~ to the Building Inspec-
tor with the following; for new buildings or new usa:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. -For existing buildings {prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of pzoperty showing all property lines, streets, buildings and unusual natural or

THOMAS 0. DARRIGAN, being duly sworn, deposes and
That he resides at 4~0 Haywaters Road, Cutchogue,
That he purchased said premises from CARL L.
WEIDINGER, now deceased, on the 19th day of September, 1969.
On information and belief, your deponent was
informed by the said late CARL R. WEIDINGER that he
constructed the house which is presently known as gAO
Haywaters Road, Cutchogue, New York prior to April, 1957,
and resided therein until he sold the premises to your
deponent in 1969.
Your deponent has now contracted to sell the
premises and has been informed that no certificate of
occupancy has ever been issued therefor.
The premises were and are improved by a one story
frame dwelling house, patio and carport, all of which are
shown on a survey made of the premises by R. Van Tuyl dated
April 20, 1985, and the same have to your deponent's
information and belief, always been used and occupied for
strictly residential purposes.
Your deponent makes this affidavit knowing that
the Town of Southold will rely on the same and your deponent
requests that a pre-existing certificate of occupancy be
issued for the aforesaid premises.
