4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.

I, MATT T. DALY, JR., residing at 3069 Grand Blvd., Baldwin, New
York, being duly sworn, depose and say:
That I am the present owner of the premises located at 750 Three
Waters Lane, Orient, New York, and designated on the Suffolk County Tax Map
as District 1000, Section 015, Block 6, Lot 23 and am familiar with the
condition of same;
That the structures thereon have been designated, arranged and
physically occupied as a one-family dwelling;
That the condition of the existing structures on the premises are
good;
That the structures are pre-existing prior to April 23, 1957 and
they are in habitable condition;
That I make this affidavit in the knowledge that the Building Depart-
ment of the Town of Southold will rely on the accuracy thereon in issuing a
certificate of occupancy.
