Upon receipt of an application for a Pre-existing Certificate
of Occupancy, I made an inspection of this two-story, wood-
framed dwelling. With Mr. Ovsianik's permission I was admitted
to building by Ms. Terry Everett and began this inspection at
approximately 1:15
The first floor has a kitchen, dining room, two living
rooms, bedroom and inside bathroom. Second floor has six bed-
rooms and a half bathroom. Heat is furnished to all rooms on
first floor except north living room and bedroom. Oil-fired,
forced warm air furnace is located in partial cellar of masonry
red brick foundation. Accessory structures total eight (8) in
number.
The following violations of the Housing Code, Chapter 52,
Town of Southold, were found:
Bathroom:
first floor, inside, no mechanical ventalation
Article II, Section 52-26 B 4 (b). No moisture
resitant floor covering at base of toilet,
Article II, ~.Section 52-26 A 4 and Article
III, Section 52-32 D. Ceiling and walls,
loose and missing plaster, Article III, Section
52-32 C. Broken electrical wall switch, Art-
icle V, Section 52-56 A 1.
Kitchen:
entry, no means of controlling light in room
at point of entry, Article V, Section 52-56 B 2.
Bedroom:
center east side on second floor, room 160 sq.
in floor area, one window totals approximately
4'-0" sq. ft., requires additional 12'-0" of
window area, Article II, Section 52-24 C 1-3.

Cellar:
foundation open on southwest corner, Article VI,
Section 52-63 B. Cellar door deteriorating -
stairs to cellar from exterior, treads are loose
and uneven. Stair well of foundation insuffi-
cient height, grade pitched toward building,
water runs into cellar, Article III, Section
52-30 A B - Section 52-31 A. Buildings sewage
drainage system, fresh air vent at house trap
terminates within building, Article V, Section
52-51 C 8.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
