Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. I am the spouse of the owner of the premises situate at 27920 Main Road, Cutchogue, New York, and as more particularly described in the annexed photocopy of survey of premises dated June 14, 1967. 2. That I make this affidavit in an effort to secure a Certificate of Occupancy in a pre-existing dwelling, to wit: Premises at 27920 Main Road, Cutchogue, NY. 3. The aforesaid premises and improvements thereon were purchased in the year 1967. 4. That upon information and belief, the premises have been used without change, except for the additions of a Bilco basement door and fences and for no other purpose than as a two-store coa~ercial building since the date of their purchase. 5.
