CREATO and MARY ANN CREATO for a Pre-Existing Use Certificate of Occupancy AFFIDAVIT STATE OF NEW YORK ) COUNTY OF SUFFOLK ) SS. : STANLEY SLEDJESKI, being duly sworn, deposes and says: I reside at 2760 Ruth Road, Mattituck, New York, and I have lived in the Town of Southold for over sixty years. I am familiar with the premises now owned by David C. Creato and Mary Ann Creato~ his wife, located at Cox Neck Road, Mattituck, New York, identified by SCTM #1000-106-8-51, as shown on the attached survey of Young & Young, as prepared for Estelle J. Olsen, dated May 15, 1972, Job No. 331. I lived on these premises from 1922 into the mid 1940's. The above-mentioned survey for Estelle J. Olsen shows a two- story frame house, a barn, two sheds and two pump houses. All these structures were on these premises during the time I lived here, well before zoning was adopted in the Town of Southold in April of 1957. These buildings are of the same size and in the same position and location as when I lived on the premises, well before April of 1957. This affidavit is given to induce the Building Department of the Town of Southold to issue a pre-existing use certificate of occupancy for all of the structures on the premises based upon the truth of the statements made herein. ~r~before me this
