Following our marriage, we bought our home and my husband went into business for himself; Other people hired by Mr. Corwin then lived in the apartment through the years; To the best of my knowledge the apartment has been in used continuously as an apartment since 1935 (except for World War II) until the present. Nav. ~ I~ ~ STATE OF NEW YORK: COUNTY OF SUFFOLK: My grandfather, Ambrose D. Corwin, built a house and garage with an apartment above it during the years 1934-1935 on a parcel of land tax map #1000-123-5-35 in Mattituck, NY, in a development known as Camp Mineola. In June 1937 he purchased a parcel of land directly North of the above parcel but separated by a 30 foot right of way. The tax map # on this parcel is 1000-123-5-14. In the latter part of 1937, he moved the garage/apartment from the first mentioned parcel to the second. Although too young to remember, it is my understanding that various employees of my grandfather lived in the apartment. In 1952 or 1953, I'm not sure which year, my mother had fallen and my father hired a lady to do the housekeeping and cooking. Her name was Helen Norman. She lived in the apartment while working for my parents. She remained with the family until 1959 when she became ill. At that time I had just graduated from college and was working out of the area. I do know that other ladies were hired to replace her and that they in turn lived in the apartment. In 1975, I purchased both parcels from my father. At that time he had hired a Nicholas Sparling to look at~er the property as they had purchased a home in Southold, New York. When I purchased the properties, my father asked me to honor his arrangement with Mr. Sparling who remained in the apartment until March 1977. At which time, I rented it to a Linda Murdoch. It has remained rented to the present time. Dated: November d'-/, 1996 'n, Jrt~/ Sworn to before me this ~O'7,p~- day of November, 1996 ELIZABETH A. FINNE NOTARY PUBLIC, .State of ~ YOlk ~11 R~~_ O'BRIEN P.E. CON~IULTINI3 ENr=INEI;RiNQ SERVICE;8 December 23, 1996 Mr. Gary Fish, Building Inspector Town of Southold Building Department P.O. Box 1179 Southold, New York 11971 Michael Corwin Camp Mineola Road Mattituck, New York 11952 Dear Mr. Fish: On December 21, 1996, at the request of Mr. Michael Corwin, I performed an on-site inspection at the above-captioned site to determine remedial measures to be taken to insure the structural integrity of the first floor support system under the living area. Specific recommendations are as follows: (1) Install new 2" x 6" joists adjacent to all existing spliced members. Notch existing 1" x 2" ledger to receive the new full depth joists and to allow for attachment to the 4" x 6" header with metal joist hangers. (2) Install a new built-up 3-2" x 10" girder and 3" diameter steel Iolly columns. Note: Iolly columns to be zip-type adjustable with a welded 10" x 10" 3~8" steel base plate. The base plate is to be secured to the concrete floor slab with 3/8" x 3" nutted sleeve type concrete expansion anchors located 2" in from the corner points of the base plate. The top Iolly column support plate shall be lag bolted to the girder and adjusted to snug the girder tight to the floor joists. Any deflected joists not in contact with the girder shall be shimmed solid to the top of the girder. The 6" x 6" wood column support installed between the studs of the west wall shall be secured to the existing 2" x 6" sill and girder with metal angle brackets. Main Road, Laueel, N.Y. 1194E) Tel. 516-298-5;252 718-358-6900 -2- (3) Install plywood blocking and 2" x 6" metal joist hangers for the existing joists at the south wall per Section A-A SK-2. (4) Install new 2" x 4" studs nailed flat to the existing wall studs at the north wall. New studs to be wedged tight to the existing 4" x 6" header and existing 2" x 6" sill. Nail a continuous 2" x 6" ledger into the wall studs and tight to the bottom of the floor joists per Section B-B SK-2. (5) All new wood members shall have allowable extreme fiber stress equal to or greater than structural grade Douglas fir Fb = 1450 PSI Fv = 95 PSI. (6) Replace rotted members at right column support at garage bay #1. If you have any questions regarding the above, please feel free to contact me. Very truly yours, Robert O'Brien P. E.
