Instructions
This application must be filled in Wpewritar OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of ell buildings, property lines, streets, and unusual
natural or topographic feature~
2. Final approval of Health Dept. of water supply and sewerage dislx)sal-(S-9 form or equal),'
3. Approval of electrical installation from Board of Firs Underwritar~
4. Commercial buildings, Industrial buildings, Multiple Residences end similar buildings and installa-
tione, · certificate of Code compliance from the Architect or Engineer responsible for the buildin~
6. Submit Planning Board approval of completed site plan requirsmente where applicable.

BARBARA ANN COOPER, residing at 5500 New Suffolk Lane,
New Suffolk, New York being duly sworn, deposes and says:
Deponent is the owner with her husband, SHERWOOD M.
COOPER, of a residence and premises on the West side of New
Suffolk Lane, New Suffolk, NY, as shown on the attached
survey by Roderick Van Tuyl, P.C., dated July 11, 1986.
Deponent further says that prior to November 12, 1955,
the premises were owned by FRED GRATHWOHL, who sold it to
deponent's father and mother, HENRY C. JORDON and MAE
JORDON. Deponent's parents lived on the premises until
September 1, 1976 when they conveyed it to deponent and her
husband. Deponent, therefore, is very familiar with the
premises and states that the residence and accessory
buildings, as shown on said survey, were all constructed
before April 1956 and that no changes have been made to the
residence and the accessory buildings since that time.
Deponent makes this affidaviat to request the Building
Department of Southold Town to issue a certificate of pre-
existing use.
