cabin.

Instructions
A. This application must be filJed in typewriter OR ink, and submitted-i~=duf~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equaJ).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buiJdings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owne~ as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

being duly sworn deposes and says that;
Ruth E. ColliDs is the owner of premises as shown on map surum, yed for
Ben Jacobsen & Leon Milowski, Jr. by Otto Van T~i1 & Son dated August 12, 1958.
Prior to 1950 said premises were owned by August Gelhaus and wife, who
constructed the residence and cabin, as shcm~n on said survey. Thereafter the
premises were %ransfered to Frank E. Magor and wife; thereafter to Ben H.
Jacobsen and Millicent R. Jacobsen; and thereafter to William R. Collins and
Ruth E. Collins.
No alterations or additions were made to the premises that required a
certificate of occupancy, based upon information and belief;
Deponent makes this affidavit to request the Building Department of the
Town of Southold to issue a Certificate of Pre-Existing Use for both the house
and ~ottag~.

Ruth E. Collins, surviving spouse of William R. Collins, owner of the premises
as shown on map surveyed for Ben Jacobsen & Leon Milowski, Jr. by Otto Van Tuyl

My parents acquired the property in 1960, at which time there was a
pre-existing cabin on the property, as shown on the attached survey.
My family has made continuous use of the cabin since my parents
acquired the property. We have used the cabin on a continuous periodic basis
until last weekend, when we moved out due to the imminent sale of the property.
At no time has the cabin been unused and vacant for longer than a few months.
I make this affidavit to request the Building Department of the Town
of Southold to issue a Certificate of Pre-existing Use for the single family
dwelling and the cabin on the property.
