APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
(~)For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.

am over the age of twenty-one (21) years and reside at 40-09
Morgan Street, Little Neck, New York and am the part owner of the
real property located on the North side of Riley Avenue and the
east side of Bay Avenue, Mattituck, Suffolk, New York, said prop-
erty known as property on Riley Avenue. The other co-owners are

the main house which was constructed in the year 1940. ~uring
the years since July 1974, myself or one of the co-owners have
been living in the two houses. We have made no structural
chan~es or additions to any of the three buildings durin~ the
time we have owned the property. I was told by Mr. Arthur Brown
that the three buildings were the same as when they were built
in 1925, his father having lived there continuously up to his
death.
The said three buildings are of frame construction, and in
good condition. They are year round residences and haVe been
used as such over the years. To the best of my knowledge, there
are no building code violations against said 'three buildings.
