A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or tppogrophic features.
2. Swam statement of owner or previous owner as to use, occupancy and condition of building.~
3. Date of any housing code or safety impection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

Upon receipt of an application for a Pre-Existing Certificate of
Occupancy I made an inspection of the buildings at the above
captioned premises. I picked up the key for the dwelling at the
Floyd King Real Estate office.on the Main Road in Orient. I began
this inspection at approximately 9:50 A.M.
The dwelling is a one and one half story wood frame structure
constructed on a cement block foundation. First floor contains a
rear entry, laundry room, bathroom, kitchen with dining area, living
room, dining room, front entry hallway and a Jalousie enclosed porch
on the southeast corner of the building. The second floor contains
two bedrooms and a full bathroom. Heat is furnished to all rooms from
an oil fired warm air furnace located in the full cellar. Domestic
hotwater is furnished to all plumbing fixtures within the ~zilding
from an oil fired hotwater heater located in the cellar. Acces.sory
structures are located in'the rear yard area. There is one Wood
framed structure on the southwest corner of the property constructed
on a poured con~e slab. The second building located on.the north-
west corner of the property contains a workshop area, greenhouse on
the south end of the building and a two car garage. This structure
was added onto under Building Permit ~ 5479, dated August 26, 1971
to which subsequent Certificate of Occupancy # Z5066 was issued.
Heat is furnished to this building from a forced warm air oil fired
furnace located centrally in the workshop area of the structure.
The following violations of the Housing Code, Chapter 52, Code
oft he Town of Southold were found:
Dwelling:
Gas Cooking range - no shut off valve at unit. A~ticle V,
Section 52-52 B 2.
Front entry - to Jalousie porch enclosure - no means of
controlling light at point of entry. Article V, Section
52-56 B 2.
Interior protection -'small room off the south end of the
second floor bedroom - plaster on ceiling is loose and
cracked. Article III, Section 52-32 C.
Safet~ device - oil fired hotwater heater - no pressure
relief valve on unit. Article V, Section 52-53 I 1.
Electrical - open junction box located on the first floor
Joist to the right of ladder from the first floor closet to
cellar - a splice made in conductor without the proper Junction
box is located o~er the hotwater heater stapled against the
first floor Joist, an outlet at the westerly end of the cellar
within the vicinity of the workbench requires a plate. Article V,
Section 52-56 A 1.
Exterior protection - foundation wall on the south end of cellar
is wet at-several areas along the Joint Just above the firstt
coarse of cement blocks. Article III, Section 52-31A.
Accessory struptures -
Electrical - storage building at the southwest corner of
rear yard - there is an open Junction box where service
enters the building and there is an ixposed hot wire Just
below the fuse switch on the easterly wall. There is an
open Junction ~ust over the bench in the garage area of
the building on the northwest corner of rear yard. Article V,
Section 52-56 A 1.
Inspection completed at approximately 10:30 A.M. and the key for
dwelling was returned ko the Real Estate Office of Floyd KIn~,
