Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sawerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming usas, or buildings and "pre-existing" land uses: ~;1. Accurate survey of pzoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to usa, occupancy and condition of buildings. 3. The premises were purchased by my parents, JAMES LAWRENCE DAVIS and CHARLOTTE T. DAVIS in 1962 from STANLEY M. WICK and GLADYS W. WICK. The premises were devised to myself and my sister, BETSY DAVIS McINTYRE by the Last Will and Testament of CHARLOTTE T. DAVIS, who died on June 29, 1987. The premises were transferred to me by deed dated August 18, 1988. It is my understanding that the house was built prior to 1957. To my knowledge and belief, only one alteration was made to the premises that would require a Certificate of Occupancy, a copy of which is enclosed.
