wood framed dwelling with attached garage; cove~ed porch; an accessory shed;
permit #6476Z-C/O Z-5401 for addition; all situated in 'A' Residential Agricultural
Zone with access to Meday Avenue.

APPLICATION FOR CERTIFICATE OF OCCUPANC~
Instructions
This application must be filled in typewriter OR ink, and submitted tm ~ to the Building Inspec-
tor with the following; for new buildings or new usa:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sawerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming usas, or buildings and "pre-existing"
land uses:
~;1. Accurate survey of pzoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to usa, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premisas, or other pertinent informa-
tion required to prepare a certificate.

SARAH ANN CHRISTODOULOU, residing at 3 Hampton Court, Coram,
New York 11727, being duly sworn, deposes and says:
I am the owner of premises located at 1330 Meday Avenue, Mattituck,
New York, Suffolk County Tax Map #1000-113.00-11.00-009.000,
Lot Nos. 31, 32 & 33 on "Map of Tollewood" filed in the Suffolk
County Clerk's Office as Map No. 175, as shown on the enclosed
survey.
The premises were purchased by my parents, JAMES LAWRENCE DAVIS
and CHARLOTTE T. DAVIS in 1962 from STANLEY M. WICK and GLADYS
W. WICK. The premises were devised to myself and my sister,
BETSY DAVIS McINTYRE by the Last Will and Testament of CHARLOTTE
T. DAVIS, who died on June 29, 1987. The premises were transferred
to me by deed dated August 18, 1988. It is my understanding that
the house was built prior to 1957.
To my knowledge and belief, only one alteration was made to the
premises that would require a Certificate of Occupancy, a copy
of which is enclosed.
I make this affidavit to the Town of Southold for a Certificate
of Pre-existing use for the residence.
