Instructions
This application must be filled in typewriter OR ink, and submitted ia,,-~.-- to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.

York corporation, duly licensed by the State of New York, and

New York, is the owner of premises known and designated as and
by the address of 910 NEW SUFFOLK AVENUE, MATTITUCK, NEW YORK
11952. Said Premises having tax map designation: District 1000;
Section 114.00; Block 012.00; lot 008.00.
2. THAT said premises was purchased by C. & D. HOLDING
& MANAGEMENT, INC~, on March 17, 1983 from ELIZABETH GRABIE BOND.
3. THAT said C. & D. HOLDING & MANAGEMENT, INC.,
has from that period to present been the owner of said premises
and that no structural changes have been made to any portion of
the premises, namely, exterior or interior.
4. THAT the premises was and continues to remain a
single one-family dwelling and residence.
5. THAT the occupancy of said residence shall remain
as such, namely, a single one-family residence.
