This application must be filled in typewriter OR ink, and submitted I~ to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal--(S~9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. I know of my own knowledge that the property known as 2360 Bay Avenue, Orient, New York (Suffolk County Tax Map 26.-1-15.1) was purchased by said decedent and her late husband, Edgar W. Chapman, on or about February 16, 1953, and that they completed the construction of the residence currently situated on such property in 1954. Such property has been owned by said decedent, either jointly with her husband or, since his death, individuallL from 1953 to her death on July 16, 1986.
