Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2.Final approval of Health Dept. of water supply and sewerage disposal-iS-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.

1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.

tion required to prepare a certificate.

ROBERT L. TOOKER, being duly sworn, deposes and says:
That he is a partner in the firm of TOOKER and SMITH, the
attorneys for the Estate of Reidar G. Carstensen, deceased, who
died on January 5, 1983 and whose Will was probated on February 9,
1983. Mr. Carstensen was the owner of premises located at 1405
Calves Neck Road, Southold, New York.
That said premises were owned by decedent and his wife since
May 5, 1951, and that the house on these premises was built prior
to 1957.
That since 1957 there have been no structural alterations to
the dwelling or changes in the use of the premises.
That on November 16, 1983, a survey of the premises, prepared
by Young & Young, dated the 31st day of October, 1983, was mailed
to the Building Inspector of the Town of Southold together with an
application for a Certificate of Occupancy.
This Affidavit is made to induce the Town of Southold, through
its appropriate officials, boards or employees, to issue a certifi-
cate of occupancy, knowing and intending that the said Town, its
officials, boards or employees will rely upon and be induced to act
by reason of the representations made herein.
