Upon receipt of an application for a Pre-Existing Certificate
of Occupancy I made an inspection of this two and partial one-
half story framed dwelling.
Inspection of second floor was made on May 24, 1976 at
approximately 1:15 P.M. I was admitted to apartment by Ms.
Kenna who accompanied me during inspection. Second floor
apartment uses front entrance of building and consists of a
living room, dining room, foyer, kitchen, bathroom and a bed-
room. Kitchen has access to deck over one story section of
first floor at rear of building. Third floor half story contains
a half bathroom and one bedroom which is unoccupied and provides
access to attic space.
Inspection of first floor was made this date at approximately
10:00 A.M. Entrance to first floor apartment is at rear of
building. I was admitted to apartment by Mr. Stumph who accom-
panied me during this inspection.
First floor consists of a utility room in one story section
providing rear entry, a kitchen with eating area, dining room,
living room, bathroom and bedroom. Heat is furnished to first
and second floor apartments from an oil fired hot water
furnace located in cellar of brick foundation.
Accessory structures consist of a brick utility building
and a wood framed two car garage.
The following violations of the Housing Code, Chapter 52,
Town of Southold, N. Y., were found:
First Floor
Rea~ entry - no means of controlling light in room on
entry, Article V, Section 52-56 B.2.
Kitchen - Gas cooking range - no shutoff valve at
unit. Art. V, Sec. 52-52 B.2.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
