Catrow do here- by swear that I am the former owner of the dwelling located at 1380 Pike St. in Mattituck. The present owners are Charlotte C. and William L. Burkard, my daughter and son-in-law. I am present- ly a resident at this location. This dwelling has been occupied continuously since November 1949. This dwelling is presently fit for occupancy. A survey was conducted by Young & Young of River- head for this property on Apr. 3, 1985 (copy included). There have been no changes to the permanent structu~s indicated on this survey, nor have there been any additional structures erected. Permanent structures on this property consist of a two-story wood frame one family home and a wood frame tool shed. The place- ment and dimensions of these structures are on the survey. The dwelling is used solely as a place of residence. I have been a continuous resident of this dwelling since its construction. Velma P.
