APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted aa~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957}, Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Gentlemen:
This statement is provided in accordance with your Certificate
of Occupancy application procedures. I Velma P. Catrow do here-
by swear that I am the former owner of the dwelling located at
1380 Pike St. in Mattituck. The present owners are Charlotte C.
and William L. Burkard, my daughter and son-in-law. I am present-
ly a resident at this location. This dwelling has been occupied
continuously since November 1949. This dwelling is presently fit
for occupancy. A survey was conducted by Young & Young of River-
head for this property on Apr. 3, 1985 (copy included). There
have been no changes to the permanent structu~s indicated on this
survey, nor have there been any additional structures erected.
Permanent structures on this property consist of a two-story
wood frame one family home and a wood frame tool shed. The place-
ment and dimensions of these structures are on the survey. The
dwelling is used solely as a place of residence. I have been a
continuous resident of this dwelling since its construction.
