Upon receipt of an application for a Pre-existing Certificate
of Occupancy, I made an inspection of this two story framed
dwelling. I picked up key from the North Fork Bank and Trust
Company, Trust Department, and began inspection at approximately
11:20 A.M.
The first floor consists of a kitchen, half bathroom in
west entry vestibule, living room, dining room and bedroom.
Heat is supplied to building from a warm air furnace with
heat outlet in floor of living room, approximately centered
in building.
Second floor has two bedrooms and a full bathroom. Heat
is furnished through grilled opening in hallway floor which
is directly over warm air register in first floor room below.
Opening is approximately 8 inches in diameter.
Foundation of building is brick masonry providing a full
cellar containing oil fired, warm air furnace; gas fired hot
water heater; electric domestic water pump and supply tank; and
a laundry sink.
A two car garage is located in rear yard at east property
line. There are no other accessory structures on premises.
The following violations of the Housing Code, Chapter
52, Town of Southold, were found:
Kitchen - gas cooking range - no shutoff valve
at unit. Article V, Sec. 52-52 B.2.
Dining Room, Living Room & Bedroom lamp cord outlets.

Exterior - front lower course of shingles - two
broken. Article III, Section 52-31 B.
Accessory - two car garage - north side, broken
window pane; siding, bottom board rotting,
grade against siding at northeast corner.

Inspection completed at approximately 12:05 P.M. Key
was returned to Trust Department of North Fork Bank.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposol--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare o certificate.
