Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to usa, occupancy and condition of buildings. 3. BROWN, being duly sworn, deposes and 325 Horton's Lane, Southold, N.Y., and that he Southold Town for the past /~years; says that he resides at has been a resident of THAT he and his wife, Carol R. Brown, own that certain property known as 325 Horton's Lane, Southold, New York and designated on the Suffolk County Tax Map as: Dist. 1000, Sec. 63, Block 5, Lot 17. A copy of a survey of these premises, dated April 21, 1987, prepared by Roderick Van Tuyl, P.C., Licensed Land Surveyors is attached hereto. THAT on personal knowledge, information and belief, your deponent can state that the structures shown on said survey, with the exception of a one-story addition to the rear of the dwelling for which Building Permit #7251Z was issued on May 14, 1974, and, subsequently, Certificate of Occupancy #Z10349 was issued on January 15, 1981, were built prior to April 23, 1957; that there have been no changes to said structures, other than normal maintenance, since prior to April 23, 1957.
