APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
This application must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

LOUISE FOSTER, residing at 1 Aldersgate, Apt. 519,
Riverhead, New York 11901, being duly sworn, deposes and
says:
I am familiar with the premises located on North Road,
Cutchogue, New York, Suffolk County Tax Map #1000-101.00-
01.00-013.000, as shown on the enclosed survey.
The premises was purchased February 14, 1928 by my
father, William H. Brown from DeForest Horton and mortgaged
by Howard Tuthill. The house was built in the early 1930's
by my parents, William H. & Jennie Brown. The garage was
built in 1973 and Certificate of Occupancy #Z5188 is
attached hereto.
To my knowledge and belief, no other alterations were
made to the premises that would require a Certificate of
Occupancy.
I make this affidavit to the Town of Southold for a
Certificate of Pre-Existing Use for the residence.
