This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposol--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. R-1 Tax Roll: Peter Bogovic & wf occupied: owner Upon receipt of an application for a Pre-Existing Certificate of Occupancy I made an inspection of the buildings on this premises. Accompanied by Peter Bogovic I began this inspection at approximately 10:00 A.M. The dwelling is a one story wood framed structure constructed on a cement block foundation and consists of a two car attached garage, dinette, kitchen, front entry vestibule, living room, bathroom and two bedrooms. Heat is furnished to all rooms from an oil fired hot water furnace located in full cellar. Cellar is partitioned with recreation and work shop area. A mud room and bath was added under Building Permit #4591Z, December 1969 to which subsequent Certificate of Occupancy #5334 was issued. A metal accessory storage building is located in rear yard area. The following violatior~of the Housing Code, Chapter 52, Code of the Town of Southold, N.Y. was found: Entry-rear, to kitchen, no means of controlling light in room at point of entry.
