This application must be filled in typewriter OR ink, and submitted ~ ~ to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of properW with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings. Industrial buildings. Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Bodenstein for a Certificate of Occupancy for property known as 6135 Indian Neck Lane, Peconic, New York With regard to the captioned matter, I, Eleanor J. Bodenstein, to my personal knowledge and through certain papers in my possession, do hereby depose as follows: The unimproved property was purchased on 6/17/b~ by Daniel C. Kluge from Irving R. Wiles and Gladys Wiles Jepeon. The Kluge family subsequently built 2 small summer cottages thereon, one on a 50' strip running approximately ~7' south from Richmond Creek, the other a 3Aroom shell on a 100' strip running thence approximately 225' south to Indian Neck Road. My parents, Andrew Clifford and Grace E. Smith. purchased the property with structures from the Kluge family on 4/4/62. In 1968 under Permit 3943Z dated 7/8/68 for a 14' x 17' and a further 12' xx 24' extension, my husband and I built the 14' x 17' addition on the west side of the cottage on the 100' portion of the property. A storage shed was built on the 50' portion in 1969 under Permit 4~8~Z dated 10/3/69. In July 1971 Alfred Goldsmith built a rear 12' x 12' and a front 14' x 14' extension on the waterfront cottage of Mr. & Mrs. Smith. In October 1971 William Jacobs added a 14' x 24' room with cellar to the west side and a 4' x 9' extension to the east side of our house o We have no record of the permits obtained by the two builders.. On 8/7/72 the deed was changed to show Andrew C. and Grace E. Smith and Albert J. and Eleanor J. Bodenstein as joint tenants of the whole property. Under Permit 2980Z dated 11/9/73 an 18' xx 26' garage was placed in the front yard area of the roadside house. On 10/~/~3 we were denied a C.O. on our house in part because of the unsatisfactory cellar stairway, which we now wish to re-build and etloloIle. On 10/21/75 the property vas placed solely in the na~es of Albert J. and Eleanor J. Bodenstein. ELEANOR J.
