I arrived at approximately 9:10 A.M. and was given the key to padlocked rear door by Mrs. John Flemming who accompanied me on the inspection. No central heating Section 512, 513a Kitchen - Unapproved cooking facilities, no refrigerator - Section 530, 531a. No light switch at entrance door from outside to control inside light - Section 529b. Rear Stairway to second story - no light switch or light for safe accent or decent - Section 215C. No Plumbing Facilities - Section 505a, b. Bedrooms, second story - Cracks in ceilings and holes where plaster let go - Section 303c. Three rooms second story ceiling under 7'6" (Violation Town Zoning Ordinance, Article I, Section 101, definitions floor area livable) General interior condition unclean and unkept Section 603a. No screens on windows and doors Section 604b, c. Exterior - Front porch deteriorating, facia, and eaves rotting badly need of painting Section 30lb, c. Opening in foundation - Section 302a. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage dispcsal--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. 5. For existing buildings (prior to April 19§7), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. C. Fees: 1. Certificate of occupancy $5.00 2.
