The Planning Department is in receipt of an application to co-locate on an existing wireless
facility and related equipment at the existing site referenced above.
Pursuant to §280-74A(d) of the Town Code, the Planning Board will hire a Technical
Consultant to aid in the review of the application and hereby requires the submission of
$4,000.00 into an escrow account with the Town Comptroller's Office to cover the cost of
the Technical Consultant's review.
Please remit a check in the amount of$4,000.00 payable to the Town of Southold. Any
unused portion of the Technical Consultant's Fee will be returned to you.
Our consultant is Cityscape Consultants, Inc. They may contact you directly regarding this
application. If you have any questions regarding this application or its process, please do

Dear Ms. Lanza,
CityScape agrees to provide a review and analysis for a wireless application submitted by
Verizon Wireless to collocate on an existing wireless facility located "for proposed new tower
construction located at 9245 Main Road,East Marion, SCTM#1000-31-3-11.31 (Application).
Review services for the Application will be performed at an agreed upon rate of four thousand
dollars($4,000). There will be no additional costs or fees associated with the review of this
Application and/or follow-up letters relating to this Application.
Consultant will provide the services as outlined in Exhibit A, Scope of Services, of the
Agreement referenced above.

reviewing and evaluating the application, including the construction and modification of the
site, once permitted, and any site inspections.
Please provide an estimate for the cost of your consulting services to determine whether the
application meets the General Requirements in the Southold Town Code for maximum
permissible exposure. Should the Planning Board approve your estimate, a purchase order

e.No building shall be occupied or used in whole or in part for any purpose what so ever until the Building Inspector
issues a Certificate of Occupancy.
f. Every building permit shall expire if the work authorized has not commenced within 12 months after the date of
issuance or has not been completed within 18 months from such date. If no zoning amendments or other regulations affecting the
property have been enacted in the interim,the Building Inspector may authorize, in writing,the extension of the permit for an
addition six months. Thereafter, a new permit shall be required.

A UTORIZATIONAND CONSENT
WHEREAS, New York SMSA Limited Partnership d/b/a Verizon Wireless ("Verizon
Wireless"), having a principal place of business located at c/o Amato Law Group, PLLC, 666
Old Country Road, Suite 901, Garden City, New York 11530, desires to install and operate the
proposed public utility wireless communication facility (the "Communication Facility") on
certain real property located at 9245 Main Road, East Marion, in the Town of Southold, County
of Suffolk, State of New York, and known as and designated as District 1000, Section 31, Block
3, Lot 11.031 (the "Property"), owned by the East Marion Fire District(the "Owner"), and
WHEREAS, Verizon Wireless, in order to install and operate the proposed
Communication Facility, must submit to the Town of Southold (the "Town") certain zoning
applications and/or building permit applications.
WHEREAS, the Owner agrees and consents to Verizon Wireless' submission of all
necessary zoning and/or building permit applications to the Town, as well as any other
application required by any federal, state or local entity with jurisdiction thereof, such that the
required approvals for the proposed installation and operation of the proposed Communication
Facility are necessary.
NOW, THEREFORE, the Owner hereby authorizes, consents and agrees to have Verizon
Wireless act as its agent, as follows:
I. The Owner hereby consents to the filing of all necessary applications by Verizon
Wireless with the Town, and any other necessary federal, state or local agency,
with respect to the installation and operation of the proposed Communication
Facility on the Property.
2. Verizon Wireless shall have the authority to act as the limited Agent for the
Owner, thereby permitting Verizon Wireless to execute and complete any
necessary documents on behalf of said Owner as may be required in order to
obtain the necessary approvals and permits to install and operate the proposed
Communication Facility.
3, This Authorization shall be limited solely for the purpose of permitting Verizon
Wireless to obtain the necessary approvals and permits to install and operate the
proposed Communication Facility, and for no other purpose.
4. This Authorization shall terminate, and be of no further force and effect, only
after Verizon Wireless receives the necessary Town, federal, state and/or local
approvals and permits required to install and operate the proposed
Communication Facility.

A UTHORIZATIONAND CONSENT
WHEREAS, New York SMSA Limited Partnership d/b/a Verizon Wireless ("Verizon
Wireless"), having a principal place of business located at c/o Amato Law Group, PLLC, 666
Old Country Road, Suite 901, Garden City, New York 11530, desires to install and operate the
proposed public utility wireless communication facility (the "Communication Facility") on
certain real property located at 9245 Main Road, East Marion, in the Town of Southold, County
of Suffolk, State of New York, and known as and designated as District 1000, Section 31, Block
3, Lot 11.031 (the "Property"), owned by the East Marion Fire District (the "Owner"), and
WHEREAS, Verizon Wireless, in order to install and operate the proposed
Communication Facility, must submit to the Town of Southold (the "Town") certain zoning
applications andJor building permit applications.
WHEREAS, the Owner agrees and consents to Verizon Wireless' submission of all
necessary zoning and/or building permit applications to the Town, as well as any other
application required by any federal, state or local entity with jurisdiction thereof, such that the
required approvals for the proposed installation and operation of the proposed Communication
Facility are necessary.
NOW, THEREFORE, the Owner hereby authorizes, consents and agrees to have Verizon
Wireless act as its agent, as follows:
1. The Owner hereby consents to the filing of all necessary applications by Verizon
Wireless with the Town, and any other necessary federal, state or local agency,
with respect to the installation and operation of the proposed Communication
Facility on the Property.
2. Verizon Wireless shall have the authority to act as the limited Agent for the
Owner, thereby permitting Verizon Wireless to execute and complete any
necessary documents on behalf of said Owner as may be required in order to
obtain the necessary approvals and permits to install and operate the proposed
Communication Facility.
3 This Authorization shall be limited solely for the purpose of permitting Verizon
Wireless to obtain the necessary approvals and permits to install and operate the
proposed Communication Facility, and for no other purpose.
4. This Authorization shall terminate, and be of no further force and effect, only
after Verizon Wireless receives the necessary Town, federal, state and/or local
approvals and permits required to install and operate the proposed
Communication Facility.

INSTRUCTIONS
a. This application MUST be completely filled in by typewriter or in ink and submitted to the Building Inspector with 4
sets of plans, accurate plot plan to scale. Fee according to schedule.

integrity of the above mentionedtower. This analysis has been performed in accordance wit°h the Crown Castle
Structural 'Statement of Work' and the terms of Crown Castle Purchase Order Number 719109, in accordance
with application 247537, revision 5.
The purpose of the analysis is to determine acceptability of the tower stress level. Based on our analysis we
have determined the tower stress level for the structure and foundation, under the following load case, to be:

1) INTRODUCTION
This tower is a 109 ft Monopole tower designed by PENNSUMMIT TUBULAR, LLC in November of 2007. The
tower was originally designed for a wind speed of 90 mph per TIA-222-G.
2) ANALYSIS CRITERIA
The structural analysis was performed for this tower in accordance with the requirements of TIA/EIA-222-F
Structural Standards for Steel Antenna Towers and Antenna Supporting Structures using a fastest mile wind
speed of 85 mph with no ice, 37.6 mph with 0.75 inch ice thickness and 50 mph under service loads.

3.1) Analysis Method
tnxTower(version 6.1.4.1), a commercially available analysis software package, was used to create a
three-dimensional model of the tower and calculate member stresses for various loading cases.
Selected output from the analysis is included in Appendix A.
3.2) Assumptions
1) Tower and structures were built in accordance with the manufacturer's specifications.
2) The tower and structures have been maintained in accordance with the manufacturer's
specification.
3) The configuration of antennas, transmission cables, mounts and other appurtenances are as
specified in Tables 1 and 2 and the referenced drawings.
4) When applicable, transmission cables are considered as structural components for calculating
wind loads as allowed by TIA/EIA-222-F.
5) At the time of analysis CCI Sites had documentation on (2) different types of foundations, a
spread footing and a caisson. Both foundations were analyzed with the worst case scenario
reported in Table 6 of this report
This analysis may be affected if any assumptions are not valid or have been made in error. Paul J.
Ford and Company should be notified to determine the effect on the structural integrity of the tower.
