LOSS CONTROL PROGRAM EXECUTIVE SAFETY COMMITTEE
MEETING MINUTES
Mon., April 6, 2015

Approval of Minutes
Approved Minutes of June 18, 2014.
Welcome and Introductions
Phillip Beltz welcomed Laurie Noonan, Manager Risk Services, NYS Municipal Workers’
Compensation, John Johnston and Jon Shearin of the Roy Reeve Insurance Agency.
Loss Analysis Report
Laurie Noonan provided the committee with a Power Point Presentation of the Risk
Management Activities for 2014, which will be e-mailed to Phillip Beltz for distribution to
Department Heads.
Some of the highlights included Southold’s Loss History. There were approx. 80 claims
last year and over 40 involved the Police Dept. The highest repeat claimants came from
the Police Dept. as well. Slips and Falls was the highest type of claim. Supervisor
Russell continues to meet with employees and Department Heads to ensure appropriate
action was taken as part of the Commitment to Safety Improving Risk Management.
Accident investigations forms are required for all accidents and near misses. Laurie
Noonan recommended a return to work/duty requirement. Councilman Ruland noted that
slips and falls are most likely to occur during the winter months and occur out in the field.
He suggested the Town look into traction footwear, such as Yak Tracks. John Shearin

noted that there is a huge benefit to issuing mandatory footwear. Supervisor Russell
noted that tick bites are becoming a huge issue with substantial cost implications. Phillip
Beltz noted mandatory training includes Lyme Disease tick bites.

Update MSDA Global Harmonization Implementation
Laurie Noonan indicated that the safety data sheets are expected to be updated by June
2016. Phillip recommended that Laurie meet with the Departments Heads that would be
directly impacted.
Review of 2014 Workplace Safety Surveys-Development of Recommendations
Approximately 281 safety surveys were submitted by the employees. Karen McLaughlin
suggested the employees be required to include their names on the surveys so that the
department head was aware of their concern and address it accordingly. Phillip
suggested they remain anonymous and include a comment as to whether or not they
spoke with their department head. Councilman Ruland will address specific personnel
issues associated with the surveys with the employee(s).
2015 Action Items
Mandatory Training (Workplace Violence Awareness & Prevention, Sexual
Harassment, Bloodborne Pathogens)
Mechanics shop asbestos will be addressed with the new building.
Phillip Beltz, Lloyd Reisenberg and Councilman Ruland will continue to work on
surveillance for the town facilities. Lloyd has received requests to record the
Town Board work sessions and a new camera system that may be a viable
solution for Town Hall. According to Jon Shearin and John Johnston, the
installation of an alarm system doesn’t change the insurance rate, however, it
does change the underwriting for bid purposes. Supervisor Russell suggested
the focus for now should be on cameras during working hours.
The building issues with the Justice Court are being addressed.
The parking lot striping is done.
Chief Flatley continues to work on the LENS program implementation.
Parks and Recreation quarterly review of equipment reporting-Ken Reeves will
be attending training this month and will implement a policy for reporting. Laurie
Noonan recommended bi-weekly reporting, rather than quarterly. Supervisor
Russell and Councilman Ruland agreed to bi-weekly reporting.

The fire proof cabinet has been delivered to the Highway Dept., in accordance
with the HCC Public Risk recommendations from 9/30/14 site visit.
Connie Solomon noted that the Annex was recently evacuated because of
possible carbon monoxide. The bank is responsible for installing smoke detectors
and carbon monoxide detectors in the Annex.
Injuries/Supervisor Accident Report
Human Resource Center-dog bite: Mitigated by Karen McLaughlin
Dept. of Public Works-neck injury: Incident Only
Dept. of Solid Waste-sprained ankle: Remand Yak Tracks
Police Dept.-hearing impaired from parade noise: Recommended Ear Plugs
Engineers-car accident due to white-out conditions: It was noted by the Vinny Orlando
that were also a couple of snow-plow truck incidents that day as well due to the white-
out conditions. Councilman Ruland commented that there is some risk you can’t
mitigate.
Highway Dept.- lack of protective eye goggles when changing hydraulic fuel: Vinny
requires the use of grinding shields.
Additional Items:
Include “lifting” into the next town-wide training session
Yak Tracks for the Human Resource Center and Dept. of Solid Waste
Mandate use of goggles.
Attempt to identify repeaters.
Supervisor Russell asked if it was necessary to redact accident reports.
