APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted ~ ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic featu res.
2.Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3.Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5.Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of pz~3perty showing all property lines, streets, buildings and unusual natural or
topographic features.
2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

severally sworn, depose and say:
1) ?hat they reside at (no ~) Main Road, Orient, New
York, 11957.
2) That they are the owners of the premises located at
(no ~) Main Road, Orient, New York, which premises is further
identified by Suffolk County Tax Map Numbers 1000; 018.00~ 02.00;
031.000.
3) 2hat they have owned the aforementioned premises
since November 22, 1978 and that during that time it has been used
and occupied solely as a one-family dwelling and that such use of
continues up to and including this date, and that the condition
the main residence and garage with attached shed on the premises
can be described as average for their age.
4) That we are making this affidavit at the request of
the Town of Southold Building Department as part of an application
for a Certificate of Occupancy for the aforementioned premises on
a "pre-existing', basis as all improvements to the property were
made prior to April, 1957.
