SITE OPERATING PERMIT
STORAGE FACILITY FOR TOXIC AND HAZARDOUS MATERIALS
Please see the attached Permit to Operate for storage of toxic or hazardous materials. This permit must
be conspicuously posted at the facility where the toxic/hazardous materials are located. Failure to post
the permit may result in delivery operator refusing to make a delivery of toxic/hazardous materials,
including fuel oil.
The Permit to Operate does not imply that all facilities are in compliance with all applicable codes.
The owner and operator of the facility are responsible for ensuring that all storage facilities are in
compliance. All facilities are subject to inspection without notice. Violations of the Suffolk County
Sanitary Code are punishable by fines of $2,000 per day, per violation. Violations of the New York
State Petroleum Bulk Storage Code are punishable by fines of up to $37,500 per day, per violation.

This permit will expire upon the date specified or upon a change of tli6-'T , h -is permit is not transferable and is granted subject to compliance with
the provisions of Article 12 of the Suffolk County Sanitary Code and 6 *NYCRW"Part 612 - 614. This permit can be revoked upon failure to follow the
applicable provisions of these parts or comply with any special conditions of issuance. All unauthorized or accidental releases of toxic or hazardous liquids
must be reported to the Suffolk County Department of Health Services at 631-854-2501 and to the NYSDEC Regional Spill Office at 631-444-0323.
